Home health services may be appropriate if you’re recovering from surgery or hospitalization, managing a chronic illness, experiencing a decline in mobility, or need skilled nursing or therapy services at home.
How do I get started?
- Talk with your healthcare provider. Home health services require a referral from your physician or other qualified healthcare provider.
- Choose Home Health & Hospice Care. If you’re given a choice of providers, you can request HHHC.
- We’ll coordinate the rest. Once we receive your referral, we’ll verify your insurance, coordinate with your provider, and contact you to schedule your first visit.
Hospice Care
Who is hospice for?
Hospice care is designed for individuals with a life-limiting illness when the focus shifts to comfort, quality of life, and support for both the patient and family.
How do I get started?
- Talk with your physician about whether hospice may be appropriate.
- Call HHHC if you have questions. You don’t need to wait until a decision has been made to learn more.
- We’ll work with your provider to coordinate the referral and guide you through every step of the admission process.
Palliative Care
Palliative care provides an extra layer of support for people living with serious illnesses while they continue receiving treatment.
