Bringing Comfort Through Connection: HHHC’s Seasons Program for Individuals Living with Dementia

June is Alzheimer’s & Brain Awareness Month, a time to raise awareness about Alzheimer’s disease and related dementias while recognizing the unique needs of those living with memory loss and the families who care for them.

At Home Health & Hospice Care, we understand that meaningful connection remains possible even when memories begin to fade. That’s why we offer the Seasons Program, a specialized hospice program designed to support individuals with Alzheimer’s disease and other dementias through the power of sensory engagement.

Reaching Beyond Words

As dementia progresses, traditional forms of communication often become more difficult. However, familiar sights, sounds, scents, and textures can continue to evoke comfort, recognition, and emotional connection.

The goal of the Seasons Program is to improve quality of life by stimulating one or more of the five senses in ways that traditional medical care often cannot. These sensory experiences can help patients feel connected to their surroundings while reducing anxiety, agitation, and feelings of isolation.

Every Patient Has a Story

The Seasons Program begins with a conversation.

Our hospice nurses and social workers meet with family members to learn about the patient’s life story, interests, favorite activities, cherished memories, and personal preferences. This discovery process helps us understand the individual beyond their diagnosis.

Was music an important part of their life? Did they love gardening? Were they devoted to a favorite pet, hobby, or family tradition?

These details become the foundation for a personalized sensory experience.

Creating Meaningful Moments

Using the information gathered from loved ones, our team creates a customized sensory kit that remains available in the patient’s room for caregivers, volunteers, staff, and family members to use.

Items may include:

  • Family photo albums or memory books
  • Favorite music playlists
  • Scented lotions or familiar fragrances
  • Stuffed animals or comforting tactile objects
  • Books and memory prompts designed to spark recognition
  • Other meaningful items tailored to the individual

These simple tools often create moments of comfort, connection, and joy that can be deeply meaningful for both patients and their loved ones.

Enhancing Care Through Complementary Services

The Seasons Program is further enriched by visits from HHHC’s complementary service volunteers. Through offerings such as pet therapy, Reiki, and music therapy, patients receive additional opportunities for comfort and engagement.

A familiar song, the gentle touch of a therapy animal, or a calming sensory experience can help create moments of peace and connection, even in the later stages of dementia.
